Procedure to avail this card:
Submit an application form with a payment of annual fees of Rs. 275/-. Yearly renewal has to be done for continued service. Application forms are available at Indiana Hospital, Mahaveer Circle, Pumpwell, Mangalore 2

A – OPD Benefits:
- 25% discount on panel doctors consultation
- 20% discount on Laboratory Investigations*
- 20% discount on all OP procedures
- 10% discount on Radiology investigations including X-Ray, EEG, TMT, ECHO, Ultra sound, CT Scan, MRI*
- 5% discount on Pharmacy & Consumables
B – IP Benefits:
- 25% discount on hospital charges for general ward category ** (excluding doctors fees)
- 20% discount on hospital charges in single room category **(excluding doctors fees)
C – Free* Treatment upto Rs. 1,00,000/- for all kinds of accidents & trauma treatment
- Free treatment for all kinds of accidents at Indiana Hospital only.
- Maximum Rs. 1 lakh for hospitalization expenses
- Additional Rs. 1 Lakh(for nominee on death of member or permanent disability
- Rs. 50,000/- for Permanent Partial Disability
- Admission in room category
- Day care procedure / Treatment due to accident is covered
- Dental treatment due to accident covered
- Free emergency ambulance service upt to Rs. 2,000/-
